Therapist Rose Cotter (Sosie Bacon) starts experiencing frightening occurrences after witnessing a bizarre traumatic incident involving a patient, Dr. Madeline Northcott (Robin Weigert), and becomes convinced that she is being haunted by an invisible entity that only she can see. The entity appears as various menacing smiling people and seems to be feeding on trauma. As Rose's hallucinations increase in severity, she visits her former therapist and discovers a connection between the entity and a chain of suicides where the victims were last seen smiling at someone before committing self-harm.
