Mark Renton, returning to Scotland after 20 years abroad, reunites with old friends Sick Boy and Spud as he faces a heart attack in a gym and navigates his life's problems, including addiction, divorce, and impending redundancy. Meanwhile, Sick Boy continues to abuse cocaine, engages in blackmail schemes, and runs a failing pub, while Spud is struggling with heroin addiction and seeking redemption. The story takes a dark turn when Begbie escapes from prison and exacts revenge on Renton and his friends, leading to a violent confrontation that ends with the duo saving each other's lives.
