Henry Spencer, played by Jack Nance, is a man left to care for his grossly deformed child in a desolate industrial landscape after his girlfriend, Mary X, leaves him. The couple moves into Henry's one-room apartment and begins caring for the child, which struggles to breathe and has developed painful sores on its skin. As Henry tries to cope with the situation, he experiences visions of his baby, including seeing a lady who inhabits his radiator, singing in the film's iconic song "In Heaven".
