Distraught and confused after her traumatic escape from the cave system, Sarah Carter is forced to re-enter the depths of the Appalachian cave system to find her five missing companions. Accompanied by a rescue team led by Sheriff Vaines, including Juno Kaplan (Natalie Mendoza), Greg (Josh Dallas), Cath (Anna Skellern), and Dan's brother, they must navigate treacherous tunnels and avoid the terrifying creatures that lurk within. As Sarah struggles to regain her memories and confront her fears, she must also face the reality of what happened to her friends and the true nature of the monstrous creatures that haunt the cave system.
